Terms of Service

Terms of service for patients and visitors using LongevityGraph through their health care practice.

Agreement

LongevityGraph (the “Service”) is software that health care practices use to run their practice and to work with their patients. The Service is operated by Multos Annos Inc (“we”, “us”, “our”). These terms apply to you if you use the Service as a patient of a practice, or if you visit longevitygraph.ai. By using the Service, you agree to these terms. If you don’t agree, don’t use the Service.

Health care practices and their workforce are governed by the separate Provider Terms of Service and the Business Associate Agreement incorporated there, not this document.

How You Get Access

  • LongevityGraph doesn’t offer individual sign-ups. Your account is created when a practice that uses LongevityGraph invites you.
  • Your practice decides which features you can use and what it shares with you through the Service.
  • Your care comes from your practice and its clinicians, not from us. We provide the software. We don’t provide health care, and using the Service doesn’t create a provider–patient relationship between you and us.
  • Questions about your care, your appointments, your prescriptions or your bill go to your practice.

Not Medical Advice

LongevityGraph is not a medical device and does not provide medical advice. Information in the Service, including AI-generated summaries and explanations, is for informational purposes and to support the care your practice provides.

  • AI-generated content is not a diagnosis or a treatment plan.
  • Talk to your practice before you change your medications, supplements or treatments based on anything you see in LongevityGraph.
  • Do not use LongevityGraph for emergencies. In an emergency, call 911 or go to the nearest emergency room. Messages you send through the Service may not be read right away.

Your Account

  • You must provide accurate information.
  • You are responsible for keeping your login credentials secure.
  • You must be at least 18 years old to use the Service.
  • One person per account. Don’t share your credentials.

Your Information

  • Health information your practice keeps about you is part of your practice’s medical record. Your practice controls that record and is responsible for it under HIPAA and other health privacy laws. We handle it on your practice’s behalf, as its business associate.
  • To get a copy of your medical record, correct it or ask how it is used, contact your practice.
  • Information you add yourself, such as connected wearables, logged doses or symptoms, is shared with your practice so your care team can see it.
  • We use your information only to provide and secure the Service. We don’t sell it and we don’t use it for advertising.

AI Apps You Connect

You can connect an AI app that supports the Model Context Protocol (MCP), such as Claude, to your own record. Nothing is shared until you sign in and approve the connection, and you can disconnect it at any time. Information you share with a connected app is sent at your direction, and the company that runs that app handles it under its own terms. A connected app can’t order, prescribe or book on your behalf.

Acceptable Use

Don’t use the Service to:

  • Upload information that isn’t yours without authorization
  • Try to access anyone else’s data
  • Reverse engineer, scrape, or abuse the Service or its APIs
  • Get around rate limits or security controls
  • Do anything illegal
  • Upload malicious files or attempt code injection

Fees

We don’t charge patients for the Service. Your practice pays for it. Any fees for your care are between you and your practice.

AI and Accuracy

  • Parts of the Service use AI, including summaries, explanations and drafts that your care team reviews. AI output can be incomplete or wrong.
  • Biomarker reference ranges are general ranges from the lab, and your practice may set its own targets. Neither replaces your clinician’s judgment about your situation.
  • We make reasonable efforts to keep the Service accurate, but we don’t guarantee that any AI output, range or calculation is correct, complete or current.

Service Availability

  • We aim for high availability but don’t guarantee uninterrupted service.
  • We may perform maintenance that temporarily affects availability.
  • We may change or discontinue features. Your practice decides whether it keeps using LongevityGraph; if it stops, contact your practice about your records.

Limitation of Liability

To the maximum extent permitted by law, Multos Annos Inc and its operators are not liable for:

  • Health outcomes, which depend on the care you receive from your practice
  • Inaccuracies in AI-generated content, ranges or calculations
  • Indirect, incidental or consequential damages

Because patients don’t pay us for the Service, our total liability to you is limited to one hundred U.S. dollars ($100), except where the law doesn’t allow that limit.

Termination

  • You can ask your practice to end your access, and you can delete your account from Settings. Deleting your account doesn’t delete your practice’s medical record; your practice keeps that as the law requires.
  • Your access ends if your practice ends its relationship with you or stops using LongevityGraph.
  • We may suspend or end your access if you violate these terms, with notice when feasible.

Text Messaging (SMS)

If you opt in, your healthcare practice may send you conversational, care-related text messages through LongevityGraph (appointment and visit updates, medication and prescription-refill coordination, lab and result notifications, care check-ins, and replies to messages you send). We do not send marketing texts.

Opting in to text messages is entirely optional and is never a condition of purchasing, using, or receiving any LongevityGraph product, service, or care. You may decline SMS, or revoke your consent at any time, with no effect on your access to care or the Service — everything we would text you is also available in the LongevityGraph app. To opt in, you check an explicit, un-pre-selected consent checkbox and verify your mobile number with a one-time code; we send texts only after you have completed both steps.

Message frequency varies based on your care. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help; you can also manage or revoke consent in Settings → Messaging. We do not share or sell your mobile number or messaging consent to third parties. See our Privacy Policy for how mobile information is handled. Messages are transmitted via our provider, Twilio. Carriers are not liable for delayed or undelivered messages.

Changes to Terms

We may update these terms. We will tell you about material changes by email or in the app at least 14 days before they take effect. Continued use after changes take effect means you accept them.

Governing Law

These terms are governed by the laws of the State of Texas, without regard to conflict of law principles.
